The rusting 1994 Oldsmobile sitting in a driveway just outside St. Louis was an unlikely cash machine. That was until the car's owner, a 30-year-old hospital lab technician, saw a television commercial describing how to get cash from just such a car, in the form of a short-term loan.

The lab technician, Caroline O'Connor, who needed about $1,000 to cover her rent and electricity bills, believed she had found a financial lifeline.






"It was a relief," she said. "I did not have to beg everyone for the money."

Her loan carried an annual interest rate of 171 percent. More than two years and $992.78 in debt later, her car was repossessed.

"These companies put people in a hole that they can't get out of," Ms. O'Connor said.

The automobile is at the center of the biggest boom in subprime lending since the mortgage crisis. The market for loans to buy used cars is growing rapidly.

And similar to how a red-hot mortgage market once coaxed millions of borrowers into recklessly tapping the equity in their homes, the new boom is also leading people to take out risky lines of credit known as title loans.

They are, roughly speaking, the home equity loans of subprime auto. In these loans, which can last as long as two years or as little as a month, borrowers turn over the title of their cars in exchange for cash — typically a percentage of the cars' estimated resale values.

"Turn your car title into holiday cash," TitleMax, a large title lender, declares in a recent television commercial, showing a Christmas stocking overflowing with money.

More than 1.1 million households in the United States used auto title loans in 2013, according to a survey by the Federal Deposit Insurance Corporation — the first time the agency has included the loans in its annual survey.

Title loans are becoming an increasingly prevalent form of high-cost, short-term credit in subprime finance, as regulators in a number of states crack down on payday loans.

For many borrowers, title loans, also sometimes known as motor-vehicle equity lines of credit or title pawns, are having ruinous financial consequences, causing owners to lose their vehicles and plunging them further into debt.

A review by The New York Times of more than three dozen loan agreements found that after factoring in various fees, the effective interest rates ranged from nearly 80 percent to more than 500 percent. While some loans come with terms of 30 days, many borrowers, unable to pay the full loan and interest payments, say that they are forced to renew the loans at the end of each month, incurring a new round of fees.

Customers of TitleMax, for example, typically renewed their loans eight times, a former president of the company disclosed in a 2009 deposition.

And because many lenders make the loan based on an assessment of a used car's resale value, not on a borrower's ability to repay that money, many people find that they are struggling to keep up almost as soon as they drive off with the cash.

As a result, roughly one in every six borrowers who take out title loans have their cars repossessed, according to an analysis of 561 title loans by the Center for Responsible Lending, a nonprofit in Durham, N.C.

The lenders argue that they are providing a source of credit for people who are unable to obtain less-expensive loans from banks. The high interest rates, the lenders say, are necessary to offset the risk that borrowers will stop paying their bills.

Title loans are part of a broader lending boom tied to used cars. Auto loans allowing subprime borrowers — those with credit scores at 640 or below — to buy cars have surged in the last five years.



The high interest rates on the loans have enticed an influx of Wall Street money. Private equity firms are investing in lenders, and some big banks are ramping up their auto lending to people with blemished credit.

Gov. Bill Haslam may have succeeded in "threading the needle" in regard to expansion of health-insurance coverage in Tennessee. The governor is eschewing the term "Medicaid expansion," which is fine if the use of the term "Insure Tennessee" will allow it to win necessary approval of the Republican-controlled state Legislature.






What is proposed is not simple expansion of the Medicaid program in Tennessee, TennCare, but rather use of federal dollars to leverage state and private funding to increase the number of Tennesseans who have health insurance.



That expansion of coverage is the goal of those who have criticized Haslam for proceeding with expansion of Medicaid through the federal Affordable Care Act.



Based on their initial reactions to the proposal, the governor apparently has ameliorated their concerns with his Insure Tennessee plan.



At issue is the use of funds available through the Affordable Care Act to make Insure Tennessee a reality and the opposition of some legislators to any aspect of the Affordable Care Act or Obamacare as they prefer to term it.



U.S. Rep. Scott DesJarlais, who represents Rutherford County and the rest of the 4th District, is among those who have ideological blinders in regard to any support for Obamacare, and he has indicated that he is "apprehensive" about use of federal dollars.



Haslam is contending, however, that Insure Tennessee is unique to the state, and if it does succeed, it could become a model for other states.



DesJarlais does not have to endorse the program, but he should give the governor an opportunity to make his case to legislators without becoming a voice against it.



Haslam has indicated that one of his concerns in expanding insurance coverage in Tennessee is the burden to taxpayers in regard to state revenue.



The governor indicates that hospitals in the state will fund any additional costs of the program, and the Tennessee Hospital Association already has endorsed the proposal.



This provides an excellent example of the use of federal dollars to leverage state resources. Hospitals should receive a good return on any investment they make in Insure Tennessee.



We congratulate the governor for bringing this proposal forward, and we urge all of the legislators to take a pragmatic rather than an ideological approach in their consideration of the proposal.



Will more constituents in their districts be able to receive health-insurance coverage or better coverage because of Insure Tennessee?



Will their constituents' health be better because of greater access to medical services through Insure Tennessee?



Will medical facilities in their districts be on a stronger financial footing because of the insurance coverage for more of their constituents through Insure Tennessee?





If they can answer "yes" to these questions, then their votes on implementing Insure Tennessee should be "yes." Source

"When you have ranking systems like this, for us to move up one, (it) means someone else moved back one," said Lillian Smith, the assistant dean for practice and community engagement at the University of South Carolina's Arnold School of Public Health. "Does that mean that we improved or someone else got worse? You've got to take these things with a grain of salt."






The report, which is released every December, is sponsored by United Health Foundation, the American Public Health Association and Partnership for Prevention. Hawaii earned the top spot in this year's rankings. Mississippi was named the least healthy state. Most states in the bottom 10 are in the South.



Nationally, there is also room for improvement. Obesity rates and the percentage of adults who aren't physically active increased between 2013 and 2014. Meanwhile, infant mortality and the number of Americans who are smoking decreased slightly year over year, while the percentage of adolescents who were adequately immunized increased.





"We applaud hard-won advances in several key measures, including smoking prevalence, even as this year's America's Health Rankings is a solemn reminder that we have a lot more work ahead of us," said Dr. Reed Tuckson, senior medical adviser for the United Health Foundation, in a prepared statement.

COLUMBIA, SC — South Carolina shoppers on the federal health insurance marketplace have more choices than last year and average prices haven’t gone up, according to statistics released Thursday by the U.S. Department of Health and Human Services.






Those statistics match similar numbers released last month by the S.C. Department of Insurance. The plans offered through the federal exchange have to be approved by the state agency.



The average number of plans available in a county in South Carolina is 59, up from 25 last year. Shoppers in some counties have more choices. You can window-shop without going through the application process at www.healthcare.gov.



The four companies that offered policies in South Carolina last year – BlueCross BlueShield of SC, BlueChoice HealthPlan, Coventry One and Consumers’ Choice Health Plan – returned and nearly doubled their plan options. One new company joined the S.C. market – Assurant Health – but its premiums are much higher than the other four companies.



Plans are offered at various levels – platinum, gold, silver, bronze and catastrophic – based on what percentage of your health care they cover. The federal statistics compared prices for the second-least expensive silver plan in each state in 2013 and 2014. In South Carolina, the average premium for that benchmark plan remained the same. Overall, premiums for all plans increased less than 1 percent, according to the state insurance department.

COLUMBIA, South Carolina — South Carolinians have until Monday to sign up for health insurance through the federal exchange if they want coverage to start Jan. 1.






The deadline applies to both the uninsured and current Healthcare.gov customers. While the second year of enrollment under President Barack Obama's signature health care law continues through Feb. 15, the deadline is important for people who want insurance — or a new plan — to take effect on New Year's Day.



Current customers who do nothing will be automatically renewed in the plan they have now on Jan. 1. But with premiums changing for 2015, administration officials and consumer advocates are urging people to come back and shop.



"There is still a great gap of information. Understandably, people are confused," said Patti Embry-Tautenhan, spokeswoman for BlueCross BlueShield of South Carolina, which is educating people about the deadline and their choices through three retail stores in South Carolina and two roaming recreational vehicles.



South Carolinians have more choices for 2015, with one additional company selling policies through the exchange and the state's largest insurer doubling its offerings.



"We know people value choice, and we wanted to give them as many options as we could," Embry-Tautenhan said.



The online marketplace for private insurance is intended for people who don't have access to affordable coverage through their jobs. According to the state Department of Insurance, 10 companies write individual health insurance policies in South Carolina. Five of those offer plans through the exchange, through which people can get federal subsidies to help offset premium costs.





The fifth company joining the exchange this cycle is Assurant Health. BlueCross BlueShield and its licensee, BlueChoice, as well as the health insurance cooperative Consumers' Choice — one of 24 nationwide created by the federal law — continue to offer plans statewide. Coventry Health Care, which was bought last year by Aetna, is also again offering plans.

Portland, OR: Cigna has long been a provider of short- and long-term disability (LTD) coverage for consumers and employees in various states, including Oregon. However, Cigna is also not unlike other insurance providers who have been accused of Denied Disability Claims, regardless of legitimacy of the claim. Anyone having filed an Oregon Long term disability lawsuit in Oregon would be forgiven for feelings of vindication after noting some other examples of legal hot water Cigna finds itself in.






Oregon LTD Insurance Woes Are a Day in the Life of Insurance FrustrationIn one example, a former insider of both Cigna and another entity is alleging that Cigna and the other party have colluded to increase health insurance premiums in an alleged effort to defraud Medicare and private insurance members.



In a release from PR Newswire (10/29/14), whistleblower Pete Bonewitz accuses Cigna of artificially increasing premiums through the application of incentives, strategic claims acceptance and by leveraging weakness in ICD-9 coding systems. Bonewitz also alleges that the other party - Zotec Partners - has been coding false claims to meet Cigna’s requirements.



“Together, Cigna and Zotec Partners’ unscrupulous methods have made disputing medical bills enormously convoluted with the intent of placing an unnecessary burden on members,” Bonewitz says in his whistleblower lawsuit. “This manipulative process has resulted in artificially increasing the risk of ALL (original emphasis) Cigna members.”



Bonewitz claims he was wrongfully terminated from both companies after attempting to bring his concerns to their attention. His 51-page bad faith insurance civil lawsuit was filed in the Circuit Court for Davidson County at Nashville (14c-4350, 3:14cv0096 and 1:2014cv01464).



Meanwhile, anyone considering or having filed an Oregon bad faith insurance lawsuit might be interested in knowing about another allegation that involves Cigna and another funder of health care services, Blue Shield. According to the Ventura County Star (9/26/14), plaintiffs accuse the two insurers of participating in a bait and switch scheme that allegedly left many consumers out in the cold after they signed up.





Plaintiff Kevin McCarthy, together with his wife Jane, is the lead plaintiff in a proposed class-action lawsuit that could have as many as 410,000 class participants, according to the report. McCarthy is self-employed. When he signed himself and his wife up for coverage with Blue Shield as part of the new health reforms, he claims to have duly checked the provider’s website and noted that his doctor and team of health care providers were a part of the Blue Shield network. He said his doctors also confirmed this, as did those who provided health care for his wife.

If you buy your own health insurance, Nov. 15 has some significance to you. It's the first day of open enrollment to shop for your 2015 coverage.






This is the second shopping period under the Affordable Care Act, so all the changes the law brought about could still feel new and foreign. For many people shopping — having the option to leave your plan for a better policy — was not a consideration. Now, it is.



In Oregon, there are 13 insurers in the individual market and 10 of them are offering plans on the marketplace at HealthCare.gov. Oregon Insurance Commissioner Laura Cali says that's quite a competitive market.



This state has always had good competition, she said, but it's increased even more in the last couple of years. In some states, one company dominates the market, so consumers don't have much choice.



"Ultimately, you've got all these companies fighting to get market share," Cali said. "They seek to differentiate themselves in a number of ways. The main way is price. They're making sure they're as precise as they can when they develop their rates and it also incentivizes them to try different things with their product offerings."



But to take advantage of these offerings, consumers need to be engaged and informed. That's easier said than done. In the last open enrollment period, Moda Health Plan dominated the market on Cover Oregon -- capturing about 70 percent of enrollees. Its silver plan premium for a 40-year-old in Portland was the second cheapest.



People shopped looking at premiums then, and they're still going to largely drive consumer decisions this year, Cali said. But a low premium doesn't necessarily equate to lower total out-of-pocket costs in the course of a year's worth of health care expenses. The best way to avoid surprise medical bills is to be as informed as possible about your plan's coverage.



Here are some features other than premium prices to look at when shopping this open enrollment period.



Cost sharing



A good starting point is to think back on what kind of health care services you use and how often. While you can't predict everything, you can take into account how often you go to your doctor, if there's a specialist you see regularly and what prescriptions you take. You could also take into account any procedures you might need in the next year.



Then figure out the funds you might pay on top of premiums to access those services. Coinsurance means the plan pays a particular portion of a service, and you pay the rest. Copays are fixed amounts you pay upfront for covered care. Check your prospective insurer's website for a prescription formulary and make sure your prescriptions will be covered.



Deductible



This is the amount you pay out of pocket before your plan starts to kick in. This is where you could be hit by surprise expenses if you're not careful. A low premium and a high deductible aren't necessarily advantageous. Healthy people who don't use health services much could get away with it, but it would be wise to set aside some money that could cover your deductible in case you get sick or injured.



Doctor



If you intend to continue your relationship with your doctors, make sure to check the provider directory so you're not hit by costly out-of-pocket costs at your next appointment. Contracts can change quickly, so keep in mind that the directory might not be up to date. The safest thing is to check with both the insurance company and your doctor's office.



Beyond that, you might be interested in seeing whether the plan you're looking at offers a broad network or a narrow network. Limiting the number of providers in the plan's network is one way companies are lowering costs. That's something to consider when you are eyeing a cheap plan.



Travel



If you travel a lot for work or for leisure, check to see whether the plan has an out-of-network benefit. Some plans will cover your out-of-network services at a higher cost share on your part, but others might not cover them at all.

Women across Pennsylvania are doing everything we can to support our families, but the deck is stacked against us — especially when it comes to getting the health care we need to lead healthy, prosperous lives.

The newly released 2014 Women’s Health Report Card for Pennsylvania underscores our reality: Pennsylvania is falling behind when it comes to meeting women’s health needs.



The report card ranks all 50 states on 30 measures of women’s health, including measures indicating coverage, access to care and health outcomes. Pennsylvania was ranked No. 22 and got an overall grade of C+. For health-care outcomes for women, the grade was a D.



Looking at measures of women’s health by race, Pennsylvania is doing even worse when it comes to women of color. For example: Latina women in Pennsylvania are more than twice as likely to be uninsured than women overall, and black women are almost twice as likely to have diabetes. These disparities are shocking — and unconscionable.



The report card was produced by the Alliance for a Just Society, a national policy, research and organizing network that has published pivotal reports on state and national health issues for 20 years. It was released this month in conjunction with Keystone Progress Education Fund.



The results of the Women’s Health Report Card should serve as a call to action for Pennsylvania policy makers. Women in our state are at risk, and many of them needlessly suffer. To improve women’s health, our elected leaders should start by:



• Expanding health coverage through full Medicaid expansion under the federal Affordable Care Act, not the Corbett administration’s Healthy PA plan. Accepting full federal funding to expand Medicaid would cover the highest possible number of low-income women and families.



• Resolving to make Pennsylvania a leader in eliminating racial disparities in women’s health and focusing on policies to improve health for women of color.





• Establishing strong standards for health-care networks to assure that people with health insurance can get the care they need without long delays or unreasonable travel. More community health clinics in underserved areas would help.

The Pennsylvania Health Access Network and the Partnership for Better Health hosts a free health insurance enrollment event from 2 to 8 p.m. Monday at the Bosler Memorial Library, 158 W. High St., Carlisle.

Navigators will be on hand to help provide one-on-one free enrollment assistance. Monday is the first day individuals can enroll in Healthy PA, which will make coverage available for individuals making less than $16,104 a year.



Last year, more than 300,000 Pennsylvanians were unable to get health insurance coverage because the state had not expanded Medicaid, the organizations said. This year, individuals will be able to enroll in Healthy PA — Pennsylvania’s version of expanded Medicaid.



The organizations are encouraging those who did not get coverage last year to try again since there are new health insurance options available this year.



Open enrollment in the Health Insurance Marketplace continues until Feb. 15.
